There was one post that came to mind where SteveK recommended reducing the dpi settings and that solved a problem, not quite the same as yours, but I was just wondering whether it might be an issue. I know that there is a more recent driver, but I always wait until I am certain that video driver updating is not going to cause me PD11 problems. I am running the nvidia 310.70 drivers without problems. If those ruled out, then I would GUESS that there may be some conflict with other software on your computer. I would recommend running the Windows Memory Diagnostics and a chkdsk C: /r and then chkdsk d: /r to rule out hardware issues. Other than that, I can only think of possible hardware issues e.g., bad memory or unseated, possible defects on your hard drive, etc. Mp4 would require PD12 to make changes and perhaps that might be an issue. Can you produce to H.264? I think that might be closer to the source format. You could try doing a "mediainfo" of your source files (it is freeware, just google it) and try to render to same bitrate and format. It is good software, not perfect, but for the price, it cannot be beaten, in my opinion. There should be a logical explanation for your issue because there are many satisfied PD12 users. Your computer should be more than capable of handling HD files. I can see nothing obvious in your dxdiag file that would account for the PRODUCE freezing.
